As of February 2002, we have not gotten forms from the district yet, however we do have one form that was made in-house -- it's a form for chaperons to fill out to authorize medical treatment in case of debilitating accidents where the chaperon cannot speak or communicate their own desires.  This form is handy for parents attending overnight field trips.
